The information on the following forms will be used to produce the Saint Paul Community Directory and your Access ID card. The Cashier’s window must receive the form back from you in order for you to receive your new student ID.

If you are eligible for the scholarship, complete the form and turn it in to Pam Lisle in the Registrar’s Office by the first day of the class in which you are requesting a scholarship.

Forms required for students to register, change course schedules, declare specializations, and many other functions are available from the Registrar's Office.
